Dissolution of a Business Name
For a dissolution of a business name, please complete the
Certificate of Dissolution of a Business Name form, specifying whether it is a dissolution of a business for an individual or partnership, or a dissolution of a corporation, general partnership, limited partnership, or limited liability company. This form should be filed in the Circuit Court Clerk's office only if the business name was registered before January 1, 2020.
